What is Care Management?
Care Management offers people options to remain independent and living in their own homes. We offer an array of in-home care services like bathing, dressing, cleaning and respite services to support family caregivers. A Supports Coordinator team is made up of a Licensed Social Worker and a Registered Nurse. They perform a comprehensive assessment to determine what help is needed, then connect individuals to quality services offered through our network partners. Supports Coordinators continue to assess need over time to ensure participants are receiving the services necessary to continue living independently in their homes and their communities. In order to promote and preserve the dignity and well-being of the people we serve, only those services necessary for independence and accepted by the individual will be provided.
Who is eligible?
Adults age 60+
Requires assistance with tasks of daily living in order to remain independent
Lives in Allegan, Ionia, Kent, Lake, Mason, Mecosta, Montcalm, Newaygo, or Osceola County
How does it work?
To start the referral process, contact us.
An initial screening will take place on the phone with one of our Access Coordinators to determine program eligibility.
Information like name, address, birth date, and marital status will be needed.
Income and assets will be reviewed.
In the event a person does not qualify, we will provide information on other community resources that may be available.
