Help Inform the State of Michigan - Take the State Health Assessment Survey!
In June 2019, the MDHHS Public Health Administration launched a 15-month State Health Assessment (SHA) process designed to help determine how healthy Michigan's residents are overall, what health conditions cause the greatest challenges in the state and what factors contribute to those challenges, and what existing resources can be mobilized to address the health needs within the state. MDHHS is conducting a State Health Assessment Survey with people who live across Michigan. This anonymous survey asks Michiganders what could make their community a healthier place to live.
Survey details include:
- Survey link: https://is.gd/MI_State_Health_Assessment (click on link or copy paste into your browser)
- Purpose: Capture responses from Michiganders from across all of Michigan to identify the greatest health needs that currently exist in the state.
- Time frame: We are conducting the survey from June 1 to June 30th.
